Herpes is a viral infection by the herpes simplex virus (HSV) that is transmitted through intimate contact with the mucous-covered linings of the mouth or the vagina or the genital skin. The virus enters the linings or skin through microscopic tears. Once inside, the virus travels to the nerve roots near the spinal cord and settles there permanently.

A person's first outbreak, which usually occurs two to 20 days after exposure, is called primary herpes. All subsequent outbreaks are called recurrent herpes. Recurrent herpes usually occurs in the same place as the primary lesion, because the virus has taken up residence in the affected nerve root.

Although the herpes virus is relatively easy to kill in a laboratory dish problems arises because the virus hides itself inside an apparently normal host nerve cell until it has multiplied itself and is ready to migrate. This is when an outbreak occurs and the blisters appear.

In a healthy individual  the immune system responds to primary infection by the herpes viruses, however some portion of the virus is able to "escape," traveling up peripheral nerves and lying dormant in ganglions near the spinal cord. Herpes viruses can remain dormant in the body for years after the initial, or primary, infection.  Prolonged stress, which compromises the immune system, can allow the virus to come out of "hiding" and travel back down the peripheral nerve to cause a recurrent infection. This cycle of latency and reactivation may be repeated hundreds of times but may follow dramatically different patterns from one individual to another.
