Easy Steps

Step 1: Request an application form here.

Before you fill out the application form you have to prepare a recent digital picture of yours –  full body shot. See the picture requirements and related FAQ here.

You will receive an application form by email.

Step 2: Fill out the application  and submit.

All required fields (marked with an *) must have data filled in before the application may be submitted.

You'll receive an email with confirmation and an invoice for an application and processing fee.

Step 3: Pay the application and processing fee  - $29 (pay secure on-line)

We accept Visa, Master Card, American Express, Discover, and Pay Pal.

Sorry, we do not accept checks or money orders and do not process orders by phone at this time.

You’ will receive an email confirming the estimated treatment plan along with payment options.

Allow up 2-3 business days for the product developing.

Step 4: Pay for the first month’s treatment and get your product shipped.

Notice that shipping on Thursdays and Fridays only.
