Step 3: Pay the application and processing fee - $29 (pay secure on-line) We accept Visa, Master Card, American Express, Discover, and Pay Pal. Sorry, we do not accept checks or money orders and do not process orders by phone at this time. You’ will receive an email confirming the estimated treatment plan along with payment options.
